Cleaver Bird Facility


In November of 1999, the Center opened a newly built community clinic, the Cleaver Family Wellness Clinic. The Wellness Clinic is committed to providing primary health care and related health and human services in a "dignified, compassionate, personalized, and accessible manner to those persons who have limited or no access to such care."

The Cleaver Clinic was established to specifically address the critical needs of a community that is especially vulnerable. More than 95% of our patients have incomes below the federal poverty guidelines. The Clinic complements Our Saviour Center's efforts to provide multiple services on one convenient neighborhood site.

Preventative health care and wellness are the hallmarks of the Cleaver Family Wellness Clinic. In 2009, the Cleaver Family Wellness Clinic had provided over 10,000 clinical and health education encounters.

Women's clinical health services are offered including gynecological care and family planning, cancer screenings, chronic disease management, and other women's health treatments.

Health care for men includes clinical services for general medicine, infectious disease, cancer screenings and other male health treatments. Immunizations, sports/annual physicals, STD screening, and primary health care services are provided for children and adolescents, ages 0-17.

The Cleaver Family Wellness Center provides a full calendar of educational sessions. These include one-on-one clinical education encounters, wellness workshops, parenting classes and forums including sessions on such topics as:
 
  • Diabetes Control
  • Alternate Shopping to Reduce Fat Consumption
  • Healthy Hearts and Diet
  • Exercise and Hypertension
  • Preparing Children for School (Necessary Immunizations)
  • STD Awareness
  • Family Planning Education
  • First Aid
  • Stress Management and Anger Control
  • Special teen workshops on Adolescent Health and Fitness
  • Teen Pregnancy and Substance Abuse Prevention.
  • Women's breast and cancer education forums and workshops are provided. Elementary school seminars are held for the management of chronic conditions, particularly diabetes and asthma
  • Emergency Preparedness
We are an approved provider for the following State-sponsored programs:

  • Family PACT (Family Planning and Clinical Treatment Program)
  • VFC (Vaccines for Children)
  • CHDP (Children's Health and Disability Program)
  • CDP (Cancer Detection Program) and
  • Healthy Families
  • Healthy Kids Program
Bilingual professionals including a Board Certified physician and medical support personnel staff our Clinic. Enrollment specialists are available for filing applications, answering questions, and annual re-determination for government programs.
